Participant Liability Waiver & Release of Claims

Participant Information

Assumption of Risk

I understand that participation in recreational sporting events organized by Beer League Athletics LLC involves inherent risks including but not limited to:

  • Physical injury
  • Slips, trips, or falls
  • Collisions with other participants or objects
  • Equipment-related injuries
  • Weather conditions
  • Other risks associated with recreational sports activities

I voluntarily choose to participate and fully accept and assume all risks, both known and unknown, even if arising from negligence of others.

Release of Liability

In consideration for being allowed to participate in activities organized by Beer League Athletics LLC, I hereby release, waive, and discharge Beer League Athletics LLC, its owners, employees, volunteers, sponsors, venue partners, and affiliates from any and all liability, claims, demands, damages, or causes of action arising from participation in the event.

This includes but is not limited to:

  • Personal injury
  • Property damage
  • Loss or theft of personal items
  • Medical expenses
  • Death

Medical Authorization

I authorize Beer League Athletics LLC and event staff to obtain emergency medical treatment for me if I am unable to do so myself. I understand that I am responsible for any medical expenses incurred.

Alcohol Responsibility

If alcohol is present at the event, I agree to consume responsibly and understand that Beer League Athletics LLC is not responsible for incidents related to alcohol consumption.

Rules & Conduct Agreement

  • Follow all event rules and instructions
  • Treat participants, officials, and staff with respect
  • Participate in a safe and sportsmanlike manner
  • Understand that Beer League Athletics LLC may remove participants for unsafe behavior

Photo & Media Consent

I understand that photos or videos may be taken during the event and may be used for marketing or promotional purposes by Beer League Athletics LLC.

Beer League Athletics LLC photographs and records events for promotional and marketing purposes. By completing this form, you grant permission for the use of your image and likeness.

Participant Information

Consent & Release

I hereby grant Beer League Athletics LLC, its representatives, employees, and agents the irrevocable right to photograph, record, and capture my image, likeness, voice, or appearance during events organized or hosted by Beer League Athletics LLC.

These materials may be used for:

  • Social media posts
  • Marketing and advertising
  • Website content
  • Promotional materials
  • Event recap videos

I understand that:

  • Content may appear on platforms such as Instagram, Facebook, TikTok, YouTube, and websites.
  • I will not receive compensation for the use of these materials.
  • Beer League Athletics LLC may edit or reproduce the content for promotional purposes.
  • Once posted publicly, the company cannot control how third parties share the content.

I release and discharge Beer League Athletics LLC from any claims arising from the use of these images or recordings.

If the participant is under 18 years old, a parent or guardian must sign below.
